Re: Bell only crossings
Apparently there's a bell only crossing here on Lillian Road in Johnsonville, PA:
https://www.google.com/maps/place/40%C2 ... -75.146857
It can be seen starting at about 10:25 in this video, and it uses a Safetran mechanical bell and what appears to be a WRRS bell:

Re: Bell only crossings
This crossing here I reckon may have been the only bell only crossing in Australia. Unfortunately it’s now abandoned and the bell is gone https://www.google.com/maps/place/John+ ... 8f1ea0210f
Luckily though there is a video of it in action, and it only operates just as the train reaches the crossing (for down trains anyways) http://pandora.nla.gov.au/pan/132791/20 ... a_1983.wmv

Interestingly enough, it appears that the bell was originally a Simplex bell, but is now a WCH mechanical:

2007 shows no WCH bell:

Scranton, PA (Crossing had equipment but was all stripped except for the WRRS mini bells, those DOT tags are not original though)
