Here's the two closest to where I live.

Duff Ave., Ames, Iowa

Iowa Highway 17, Jordan, Iowa

And a couple in Des Moines that are on low use spurs... both on MLK Parkway.

This crossing: The city of Des Moines originally was wanting to convince IAIS to abandon the trackage as they did not want to have to have a crossing here when constructing the MLK Parkway... having to pay for a portion on the signal cost. There are no active shippers on the spur, but IAIS does occasionally park power or store some cars.

And this crossing recently created as well on the newest portion of the MLK Parkway. The eventual goal is to extend MLK Parkway out to the 65/5 Bypass using a portion of the former NS right of way past the Titan Tire plant.
Well, :Censored:

I believe it was this one, New York Ave.

I know that thanks to Millenniumforce's video.

I don't know that I would consider those quad crossings. Yes, there are 4 gates, but that's because it's on a divided highway. My interpretation of a quad crossing means it would close the entire crossing (from both directions) when the gates are down.

Just my :2cents: worth though. So, if other's disagree that's fine.

Yea, those are called median signals. It ain't a true quad-gate set-up.
They/Them for me, please.

Way too many to list here... but the Union Pacific/Amtrak line between Chicago and St Louis will eventually have all quad crossings due to high speed rail. It's been on ongoing project for close to 15 years now. Unfortunately this has meant (or will mean) the demise of some signals that have been around since the 70's in some cases.

Here is a few close to my hometown of Springfield, IL:
