Here is one in Montevideo (the capital of Uruguay) with teardrop bells! This was found by McK&H.Aust.

I wonder why this road has never had an overpass built. Its just a open grade crossing on a freeway.

You can find quite a few crossings such as this one in central/northern Wisconsin... around the Green Bay area especially. As Zach mentioned, although the roads are 4 lanes... and in some cases have exits and entrances like an interstate would (especially in larger cities), it is not officially a freeway/interstate. Which means you can find driveways leading up to residences along with standard 4 way intersections with traffic lights in some areas.

Looks like they wanted to replace the signals but they never did.
There use to be three signals on each side!
