I can think of five in the Chicago area, two of which are operating, one is a warning bell for a crossing, and two are dead on cantilever masts.

Clinton Ave in Chicago

Itasca Ped Crossing

In Blue Island, because there are no crossing triggers, there is a manned tower for the crossing. In the tower is a mini bell that sounds when ever a train reaches with in a half mile radius of the crossing, or something like that.

Medhina Station (Roselle)
This crossing has M-bells. Two active, two dead. The two active are on the gate masts. One is a WCH M-bell, the other is an MI M-bell. There are two Mini bells though, rusted and shut off, they sit on the cantilevers at this crossing. What is extremely strange though, is that at one point there were missing! They were gone for a good month, then they came back. Nothing could explain that. I'll have pictures of this crossing soon.

Struggled to find it, but street view has one place where it shows that the Mini Bell, Classic Cantilevers and inverted SORS signs have been removed and upgraded from the Clive, IA NW 86th Street crossing.

:Cry1:

The low amount of traffic on the line certainly doesn't seem like it requires this advanced of a set-up... however with a very busy intersection with University Blvd. so close, I guess it was just a matter of time.
Well, :Censored:

*part of the downeast scenic railroad, None of them are being used now.But thats better than them being gone and a rail trail be put in  .

EDIT: As of June 2010 the Front ST. Mini Bell Was replaced with a WCH bell.
As of 12/5/11 the Signals (including the mini bell) on Wilson street were for some reason removed, although they actually were still used for the active tracks..
