That number was used by NS on the crossings they took-over from Conrail shortly after Conrail was split in 1999.
No idea how that emergency information sticker got to this crosing specifically, given that this was never a Conrail crossing, but I suspect it was likely the signal maintainer here at the time got sent that one in error.
