There used to be a couple in Dallas TX on the old MKT line, which became DGNO. It has now been reconstructed for DART, and these crossings have been changed.

I also recorded this guy in action last year:
There used to be one at Ernestine Street in Orlando, FL, but the CFRC managed to get a second gated signal installed at it.

Speaking of Ohio, here's one on Eastport Road in Scio, OH:

It appears to be due to having been part of a rather wide double-crossing, where the second crossing has long since been removed and turned into a trail:

Still can't recall where the PA one is or was, and I doubt I'll ever be able to find it.
