us-41 in L'Anse michigan might have one but the street view is from 2008 so it might be gone.

sorry for bumping this five year old post but i was cursing around street view and these hybrids are no more they street view shows that they have been replaced general signals e-bells sadly.
aug 2011 when it had it's hybrid bells!

to bad one of the smiles isn't a face with a hand next's to it's mouth saying boo with it being in one of the comic bubbles to boo the railroad because this is one instance where i would use it.
