Here's one in Creston, Ohio. Suprisingly, out of the 2 bells here; the teardrop, and a safetran mechanical bell, the safetran bell is the one that's dead. Wheeling & Lake Erie runs through this crossing 2 times a day.

There are 2 other nearby crossings that have similar relay cases but only a pinnacle on top, perhaps they also had bells in the past?
