I agree... it is easy after doing the same procedure over and over again to get complacent. I remember an accident about 10 years ago that wasn't pretty (see below link). I wouldn't want that on my shoulders.
I think that story was seen on TV, on a special called "Trouble On The Tracks", which aired on the Discovery Times channel, back when I had Dish Network.
I recorded that program, and this happened to be one of the 3 or 4 stories they talked about on that program. I have that available as one of my freebie DVDs.
