Broom Street in Madison, WI (not on the site) has 2 as well.
I saw a few in the Barrington/Arlington Heights/Mount Prospect, IL area along the UP Harvard Subdivision (a.k.a. the Metra Northwest Line).
Chicago & Northwestern started installing this type of bell shortly before they were bought out by UP, so they're not uncommon on their old lines around Iowa, Wisconsin and Illinois.

Yeah, there used to be a few on the old Ankeny Subdivision in Ankeny and Slater before UP scrapped the line.
Also can't forget the one that started it all in Eldora on Iowa Hghy 175:

(Ex CNW, Ex UPRR, now Iowa River RR)

Glen Ellyn has 2 crossing with the bell. Lombard used to have an all chrome one, but now the crossing got taken away. West Chgicago has one, Lombard has a few, Villa park has a few. Elmhurst has I think 2, or 3, left. Bellwood has one of the few M-bells on the Union Pacific Geneva Sub and an ASC White E-bell, and thats all I know of.

I like that cantilever... but crappy job on the mast lights. They have only one light pointed in each direction.

Wow...some street running in Tucson... I may have to go see that.... Is that a UP line??
