Ideally this railroad crossing should be replaced with a grade separation because it is right by a hospital, but can it be done? It seems like it would be difficult.

Yea, though you'd have to sink the freeway ramps (farther) and intersecting road in addition to the road for grade-separating the crossing in question.

FWIW, the hospital probably had the railroad tracks in mind when they were building/expanding on the property as well.
