There used to be a couple in Dallas TX on the old MKT line, which became DGNO. It has now been reconstructed for DART, and these crossings have been changed.

No longer ground-level trackage here

Still has ground-level trackage, both directions have gates now, DART is in tunnel under xing

There used to be one at Ernestine Street in Orlando, FL, but the CFRC managed to get a second gated signal installed at it.

I also remember seeing a two-way road with just one gate arm somewhere up in a tiny town in rural Pennsylvania, so I'll have to see if I can find it again.

It appears to be due to having been part of a rather wide double-crossing, where the second crossing has long since been removed and turned into a trail:
